- You are traveling to a country where your current, valid passport will not be accepted due to Visas or entry stamps -or-
- You need to travel during a period of time when your passport will be at a foreign consulate getting a visa

Important
PLEASE NOTE, YOU WILL NEED TO SEND SWIFT PASSPORT SERVICES YOUR PRIMARY PASSPORT TO OBTAIN THE SECONDARY PASSPORT. UNFORTUNATELY THERE ARE NO EXCEPTIONS.
Step 1: Gather the items listed below
Your U.S Passport
Your Passport must be in good condition. If you have less than 1 year validity we recommend you renew your passport before you obtain your second passport
DS-82 Passport Application
- Option 1: Click here to print a PDF version of this form. Complete it using black or blue ink, and sign it.
- Option 2: Complete this form online, via the Department of State website and print it.
- Enter all information requested- missing information can cause delays
- You will come to “Summary of Fees” page (only check $60 Expedite Fee & Passport Book Fee) & click SUBMIT
- on the next page, scroll to bottom and click CREATE FORM
- The completed DS-82 Passport Application form will open as a PDF document, PRINT THIS FORM
Helpful Hints
It is important that you do not leave any fields blank. If it is necessary to cross something out, strike through the word(s) 1 time and initial next to it. If you have several cross outs, it is best to start over with a new application. The information you provide should match the information on your current passport. If your name has changed since your passport was issued, you will need to renew your passport before we can obtain a second passport. Please call for more detail.

2 Identical Passport Photos
Photo Requirements: Photos must have been taken in the last 6 months, be 2 inches by 2 inches in size, and have white background with no patterns or background objects visible. Photo must be in color and be free of glare. Ears should be visible, and you must be facing forward. Dark glasses, headbands, and spaghetti straps are not permitted. Feel free to smile!
- Option 1: email a photo to Swift Passport Services photos@swiftpassport.com using the following guidelines. Swift Passport will size and print your passport photos for a nominal fee of $15.
- Photo file needs to be attached to the email as a .jpeg file. Other file formats cannot be properly formatted
- Have someone else take the picture of you; please do not send pictures you take of yourself.
- Photos need to be taken with a digital camera, phone camera photos will be of insufficient quality
- Please do not attempt to resize or format the photo in any way
- Option 2: Obtain 2 passport photos from a passport photo service (Walgreens, CVS, FedEx, etc.). Include these photos in your package to Swift Passport Services.
Proof of Departure
Please provide ONE of the following
- A photo-copy of your airline ticket or a copy of your itinerary from your travel agent, airline, or online booking company
- A letter detailing your business travel. Follow the sample "company letter of expedite" format.
- A letter explaining that you will be driving to Canada and/or Mexico. Include intended departure date, intended return date, and destination.
- If for some reason you cannot provide one of the above, please contact Swift Passport Services for further instruction.
Request for a Second Valid Passport
- A letter written by yourself or your employer explaining why you need a second passport. While the Passport Agency does not require this information, they may ask for it at any time. Including this SIGNED letter may prevent delays in processing.
- In addition to this letter please complete and sign the Request for a Second Valid Passport form.

There are 3 fees associated with this service that can be paid with all major credit cards or a money order

If you would like Swift passport to print your photos, there is a $15 fee as mentioned above Submit your order to make payment for the above fees.
Step 2: Submit Your Order
- Option 1: Submit your order online
- Option 2: Call 877.917.9438 and place your order over the phone. Placing your order on the phone takes only a few minutes. Please be prepared to give us payment and shipping instructions.
- Option 3: Click here to print an order form. Complete the form and send it to Swift Passport with your passport application materials. If you are using a 1, 2 or 3 day service please call to place your order or fax us the order form (Fax 312-268-6237) prior to shipping. Failure to do so may delay your service.
Step 3: Ship all Application Materials to Swift Passport Services
We highly recommend that you use FedEx to ship your application materials, as they are the most reliable shipping service and will provide you with accurate tracking information. Please ship your application materials to our Chicago location unless you have discussed an alternate location with our passport and visa specialists.
Swift Passport Services can email you a pre-paid FedEx airbill to use for inbound shipping upon request. Please call or email us if you would like us to email you a pre-paid airbill.

Helpful Hints
If you are using a 1 day service we will need your application by 8:30am in Chicago. It is very important that you use FedEx FIRST overnight with NO SIGNATURE REQUIRED for this service.
If you are using 2, 3, 5, 10, or 14 day services, we will need your application by 10:30 to begin processing your application the day we receive it (FedEx Priority overnight is recommended). Our office building does not accept Saturday deliveries.
