Complete the Passport Application form online, via the Department of State website and print and sign it with black ink in front of the Passport Acceptance Agent.

- Enter all information requested- missing information can cause delays
- The printed application must contain a 2D barcode in the upper left corner Handwritten applications ARE NOT accepted by the US Passport Agency
- All applications require an original signature, e-signatures are not permitted
- Note that the new passport could be shipped back to the address on the application directly from the US Passport Agency. Please make sure this is an address that can receive UPS shipments (no P.O. Boxes).
- The completed DS-11 Passport Application form will open as a PDF document. Do not alter this PDF if changes need to be made. Please recomplete application online and print again.
- Print this form single sided, and DO NOT SIGN until you are in front of a PASSPORT ACCEPTANCE AGENT where you will sign these documents in front of an acceptance agent.
You will need to make an appointment to present documents to a passport acceptance facility to execute this application form and all of the other required documents. At the acceptance facility, please ensure to ask that the documents be sealed for HAND CARRY –DO NOT LEAVE SEALED ENVELOPE AT POST OFFICE– there will be a $35 fee to pay at the post office.
