A DS-64 Lost or Stolen Statement will automatically print when you’ve filled out the application. Be sure to include this form with all of the other application material.
Please Note: It is important that you do not leave any fields blank. Be very specific and include detail in your answers. Applications that say things like “I don’t know how I lost my passport” are frequently rejected until a more detailed DS-64 can be submitted.
If applicant is not using the hand written DS-64, please sign and date the DS-64 form in the box containing the 2D barcode
If your DS-64 did not print properly, you can print the DS-64 Lost or Stolen Statement here. Complete it using black or blue ink, and sign it.
