- The printed application must be typed in the form. Handwritten applications ARE NOT accepted by the US Passport Agency
- All applications require an original signature, e-signatures are not permitted
- Note that the new passport could be shipped back to the address on the application directly from the US Passport Agency. Please make sure this is an address that can receive UPS shipments (no P.O. Boxes).
- Print this form single sided, and sign in black ink. Do not date the signature line until the day you are ready to send Swift your application. Swift has a short window to submit your application from the day it is signed.
