It is imperative you include copies of all application materials that will be sealed inside of envelope. Applications received without copies will be delayed. When Swift receives your application material, we cannot open the sealed envelope you send and need to review the documents you send.
Swift needs copies of the following:
- DS-11 Application- You can print 2 copies, we do not need a copy of the signed form
- Swift Letter of Authorization
- Proof of Travel
- Check to the U.S. Department of State
- Proof of US Citizenship
- Copy of valid ID(s)
- Copy of passport type photo
