Submit your ACTUAL passport containing two blank facing visa pages. Your passport must be valid for at least 6 months from date of entry and must be signed. Please note that the last 2 pages in your passport ARE NOT counted as visa pages, those are amendment pages.
IMPORTANT: If the passport is only valid for one year or less, the consulates/Embassy will issue a limited visa.
