Complete this form online, via the Department of State website and print it. Hand written applications will not be accepted by the passport agency. Please pay close attention to the following instructions to prevent any delays with your new passport expediting service:

A DS-64 Lost or Stolen Statement will automatically print when you've filled out the application. Be sure to include this form with all of the other application material.
Please Note: It is important that you do not leave any fields blank. Be very specific and include detail in your answers. Applications that say things like "I don't know how I lost my passport" are frequently rejected until a more detailed DS-64 can be submitted.

One copy will go inside the sealed envelope and one should be attached to the outside of the envelope. If there is not a SECOND copy of the Letter of Authorization sent to Swift with the sealed envelope, then your service will be delayed by at least one business day.

Proof of Departure Within 2 Weeks
Note: many acceptance agents require a flight itinerary indicating that you are traveling in 2 weeks or less. If you do not have a flight itinerary showing that you depart in less than 2 weeks, we recommend calling the acceptance agent that you plan to visit to determine what they will accept in lieu of a flight itinerary (letter explaining travel, itinerary with proof of travel in more than 2 weeks, etc). Read more about proof of travel, and examples of what may be acceptable proof of travel on our blog before proceeding if you do not have a flight itinerary showing a departure date in the next 2 weeks.
When Swift receives your application material, we cannot open the sealed envelope you send. In order to streamline the process, please include photocopies of all applicaiton materials that will be sealed inside of envelope. Swift needs copies of the following:
Once you have collected the documents listed, you will need to take them to a post office or county/city clerk to get the application executed by a "passport acceptance agent." Find a Passport Acceptance Agent near you, just enter in your zip code. Swift suggests calling ahead to the Acceptance Facility to confirm that the details on this website are accurate.

Please note that if the Passport Acceptance agent prompts you to write a check for an amount other than $170, $200 or $90, the check will not be accepted, and your application will be delayed until a replacement check is provided.
Multiple applications: If you are getting more than 1 application sealed, it is important that each application is in its own sealed envelope. Multiple applications in 1 sealed envelope may be denied.

Please ensure that your package arrives by 8:30 if you would like your documents submitted the day we receive them (10:30am is too late). Please use "FedEx First Overnight” or "UPS Early AM,” to ship us your documents. All SAME DAY orders must be received by 8:30 AM.
